Individual Disability Income Products
Individual DI Non-Cancelable Series: Occupational Classes
5AP-2A
This policy is designed to provide income replacement benefits
to insured individuals working in professional, white collar and
gray collar technical occupations. This policy cannot be
cancelled as long as premiums are paid and premiums are
guaranteed as stated in the policy. The coverage is guaranteed
renewable to age 65. Beyond age 65, the policy is renewable for
life, as long as the policy was in force the prior year and the
insured is gainfully employed. Premium rates after age 65 are
based on the rates in effect for such ages.
Non-Cancelable Series: Home Provider Policy
This policy is available to all non-cancelable occupational
classes and can be used as a low cost, catastrophic type of
disability income coverage. This is not available for
Employer-Sponsored Plans. However, it was primarily designed for
qualifying at-home occupations. Beyond age 65, the policy is
renewable for life, as long as the policy was in force the prior
year and the insured is gainfully employed. Premium rates after
age 65 are based on the rates in effect for such ages.
Guaranteed Renewable Occupational Classes A, B
This policy is designed to provide some income replacement
benefits to insured individuals working in occupations such as
skilled technician, on-site supervisor, manual laborer and some
unskilled trades. This policy cannot be cancelled as long as
premiums are paid. Insureds can renew each year before age 65.
Beyond age 65, the policy is renewable for life, as long as the
policy was in force the prior year and the insured is gainfully
employed. Premium rates after age 65 are based on the rates in
effect for such ages.
Business Disability Insurance
Disability Overhead Expense: Occupational Classes 5AP-3A
This policy provides coverage designed to reimburse owners of
small businesses (sole proprietors, partners of closely held
corporations), operated from a location away form the home. For
normal and customary expenses necessary to maintain the
business. This policy cannot be cancelled as long as premiums
are paid and premiums are guaranteed as stated in the policy.
The coverage is guaranteed renewable to age 65. Beyond age 65,
the policy is renewable for life, as long as premiums are paid,
insured is gainfully employed at the usual occupation at least
30 hours per week, and insured is responsible for the expenses
of maintaining the business or office. Premium rates after age
65 are based on the rates in effect for such ages.
Group Disability Insurance
Employee-Sponsored Multi-Life Plans
Designed to provide income replacement benefits to a group of
insured individuals working in professional and executive
occupations within the same employer. A major feature of this
program is the availability of
Guaranteed Issue underwriting for groups meeting certain
criteria.
This program is not available to medical occupations. The
program can be offered on either an employer-paid or voluntary
basis.

The Purpose of Disability Insurance
Disability insurance was created to insure you for

Q. If you collect disability income benefits, do you have to pay
Income Tax
on them?
A. Provided
you pay the premiums out of your own pocket on an after-tax basis,
the benefits are not taxable. However, if premiums are deducted from
your paycheck on a pre-tax basis such as in a Section 125 cafeteria
plan, benefits will be taxable. If your employer pays part of the
premiums, only the portion of the benefit that is attributable to
your own contributions will be tax-free.
